• RN for Cooley Dickinson VNA & Hospice for more than 20 years, as a Weekend Nurse Manager, a Hospice Nurse Coordinator and a Nurse Case Manager/Visiting Clinician. As of February 2018, following recommendation of a LEAN team process, applied for and accepted position as Nurse Coordinator, Inpatient at Cooley Dickinson Hospital, which, following LEAN team analysis that identified a need for an additional layer in the hospital setting to ensure optimal coordination and transition of care from hospital to home for VNA and Hospice patients.
• Attend AM interdisciplinary rounds on medical units, collaborating directly with hospitalist team, bedside RN's,nurse case managers, and MSW's regarding potential or active VNA and hospice referrals.
• Arrange and provide inpatient hospice informational meetings
• Assist with discharge coordination for both VNA and hospice referrals
• For VNA services- assist with coordination of skilled VNA care needs, assessment, home IV therapy, wound care, wound vac, drain care and management, rehab services including PT, OT and SLP to ensure supplies and accuracy and inclusion of detailed discharge instructions. For Hospice Services, coordinate and provide hospice informational meetings, GIP inpatient hospice evaluations/referrals, ordering DME for home delivery, collaborate with CDH hospitalist provider for DC medications, ensure local scripts readily available, clear and efficient dc planning including patient and family, collaboration with VNA & hospice scheduling to ensure appropriate admission timing and delivery of services.
• Successful in improving presence, relationship building and forming alliances with inpatient team at Cooley Dickinson Hospital, with evident improvements in the process for referral management for VNA & Hospice patients.

Supervised and managed clinicians in Nursing, Rehab an Hospice throughout the weekend. Coordinated care provision for VNA and Hospice patients, from intake to processing referrals, collaborated with patients, families, field staff, vendors, pharmacies to ensure accurate, appropriate and efficient visit schedules. Provided consistent 24/7 on call support for both VNA & Hospice teams, resolving clinical issues and addressing patient concerns promptly.

• Administered medications and treatments in accordance with care plans.
• Monitored patient vital signs and reported changes to healthcare team.

Cared for a variety of patients on VNA, Palliative VNA or Hospice services. Gained large knowledge base of community nursing, developed detailed, varied and complex clinical skills, assisted with appropriate coordination of care and follow up for the patients and families in my care.
